Here are the instructions on how to complete this rebate offer by mail:
- Mail the original form, completed
- All items purchased must be on ONE register receipt
- Include your original store identified cash register receipt dated 9/22/14through 11/30/14 showing purchase of $30 or more of eligible P&G products circled
